To obtain a new patient appointment for mental health services at the Live Oaks Clinic, please do the following:

• Ask your primary care provider to fax a referral to our office at 205-972-0267
• If you do not have a primary care provider, our Christ Health medical clinics would love to provide you with this service. To establish care at Christ Health for medical care please call 205-380-9455.
