The Live Oaks Clinic provides mental health education, counseling, and medication management through the following services:
- Child/Adolescent Psychiatry (birth to senior in high school)
- Adult Psychiatry (ages 14 and older)
- Medication Management
Scheduling Your First Visit
To obtain a new patient appointment for mental health services at the Live Oaks Clinic, please do the following:

Ask your primary care provider to fax a referral to our office at 205-972-0267. If you do not have a primary care provider, our Christ Health medical clinic would love to provide you with this service. To establish medical care at Christ Health, please call 205-380-9455.

Once we receive a referral from your primary care provider, the Live Oaks Clinic will send you a link to register for the patient portal along with instructions for completing your new patient intake forms.
*You must return your new patient intake forms to our office before scheduling your first appointment.
