Our Care Model

Christ Health is recognized by the National Committee for Quality Assurance (NCQA) as a Patient Centered Medical Home (PCMH). PCMH is a model of health care that strengthens the relationship between providers and patients through long term care from a care team that is comprised of multiple health care providers and led by a physician.

Your care team’s goal is to provide personalized and coordinated care that is high quality, affordable, and effective. PCMH services provided by Christ Health include same-day and evening appointments, preventive care, school physicals, an online patient portal, and self-care management.

What do I bring to my first appointment?

When you come to Christ Health for the first time, please be prepared with the following documents:

  • Your photo ID
  • Proof of insurance
  • Money for co-pay (cash, check, and all major credit cards are accepted)
  • All medications
  • Prior year W-2 form and Federal tax return (including Schedule C for self-employment). If you have just started working and only have one paystub at the time of the visit, one paystub will be utilized for a period of 30 days from the date of the applicable visit. If no additional documentation has been provided by you within that 30 days, you will be responsible for the full amount of the visit.

If you do not have insurance, you will need to bring:

  • Proof of income*
  • Payment for your visit (check with the front office to confirm this amount)
  • All medications

*Proof of income documents include: check stub, letter from employer, letter of support from the person you are currently staying with, or bank statement. All of these proofs of income must be from the past 30 days or less. Unemployment benefits, a food stamp letter, a social security letter, or a disability letter will also work if it is less than 60 days old. If you have no proof of income, please be prepared to pay $100.

*The Live Oaks Clinic may require additional forms. Please confirm with their office before your appointment.

What should I expect on my first visit?

For new patients, please come 20 minutes early to complete your online check-in.

We provide a 10-minute grace period for patients arriving late. After the allotted grace period, Christ Health reserve the right to give your appointment to another patient on the standby list.

Co-pays are due at the time of service. Please schedule a Financial Appointment to make special payment arrangements. We accept all major insurances except Viva UAB plans. We do accept other Viva plans. If you have Viva insurance, please confirm with your insurance company and our scheduling team that your plan is accepted at Christ Health.
